Required Documentation for New Hires
The required documentation to hire faculty should be submitted to Academic Affairs.
Please note: Hiring packets will not be processed until Academic Affairs receives all official transcripts.
Faculty Documents
All of these documents should be submitted to the Office of Academic Affairs when hiring a new faculty member.
- New Faculty and Adjunct Hire Checklist
- Approved HR Action Form
- Faculty Qualification Certification Form
- Memo to Hire
- Signed by chair/unit administrator and Dean.
- Faculty Application & Letter of Interest
- Curriculum vitae
- Must include JU appointment.
- Background Check Authorization Form
- Required Screening Question
- This form should only be provided to an applicant AFTER the interview stage, provided you still have an interest in further pursuing the hiring of an individual. A blank copy of the Required Screening Question is available online.
- Letters of recommendation (3)
- Information Sheet - New Hire
- With all sections completed, including budget line.
- Original transcripts from all institutions
- Transcript Notice Letter
- If applicable. Do not use this form if transcripts are in hand.
- Copies of all licenses and certifications
- If applicable.
- Confidentiality Agreement
- Employee Benefit Document Access Acknowledgement Form
- For full-time faculty only.
- Direct Deposit Form
- Attach voided check or request form from bank.
- W-4
- I-9
- Visual inspection of documents to be certified by department associate and attached to I-9.
Adjunct Documents
All of these documents should be submitted to the Office of Academic Affairs when hiring a new faculty member.

- If the adjunct has less than a Master's degree in the discipline being taught.

- Visual inspection of documents to be certified by department associate and attached to I-9.
Additional Documentation
Documents and forms below will not be applicable to all new hires. Please review the forms below or work with Academic Affairs to determine whether additional forms are needed.
