Reporting to Jacksonville University

If someone has experienced sexual misconduct, they can report directly to the Title IX Coordinator. Reports filed through this office are separate from any law enforcement reports which may or may not have been filed. Individuals who report directly to the Title IX Coordinator will also be informed of how to file a report with law enforcement, if they choose to do so.

Employees who learn that someone has experienced sexual misconduct MUST report all information known to them to the Title IX Coordinator. See the link below to file an online report.

How to File a Report

  1. Complete the online form:  Title IX Reporting Form
  2. Contact a member of the Title IX Team
  3. Report to the Title IX Coordinator by phone at 904-256-7067 or in person on the 3rd floor of Davis Student Commons during normal business hours
