Contact the Student Health Center by calling (904) 256-8080 to report your positive
case and to schedule an appointment.
You will be evaluated by a SHC provider.
The SHC will advise you on isolation, masking, and symptom management. Your isolation
will not be determined by a number of days; it will be based on the improvement of
your symptoms as determined by the SHC.
If you live on campus, you will isolate in your current housing assignment. You will
not be relocated to a different on-campus isolation space.
You will need to wear a mask to visit the dining hall for meals. Meals will not be
delivered to you while you are in isolation.
If you need a doctor's note for any missed class due to isolation, the SHC can provide
you a note via the patient portal after you’ve been evaluated by a SHC provider.
COVID Faculty Notifications will not be sent by the Dean of Student’s Office.
Your SHC provider will advise you about informing anyone you know is a close contact
about your recent positive test results.
The University will not conduct COVID contact tracing.
If you don't feel well but haven't been tested for COVID:
Stay at home or in your current housing assignment, and contact the Student Health
Center at (904) 256-8080 to schedule an appointment.
Isolate yourself until your appointment. Stay in your room, wear a mask around others,
and only take food to go (while masked) from the dining hall.
Refer to your class syllabus for instructions on reporting an absence due to illness.
Your SHC appointment discharge paperwork can be used as a doctor's note for class
needs.
If you have been exposed to COVID
If you have been exposed to COVID but don't have any symptoms, you don't need to do
anything.
If you have been exposed to COVID and you have developed symptoms, isolate yourself
and contact the Student Health Center at (904) 256-8080 to schedule an appointment.
