Student Health Insurance

The University student health insurance policy is compliant with Federal Health Care Reform law (section 1302b of the PPACA).

All full-time Traditional Undergraduate students enrolled for a minimum of twelve (12) credit hours will automatically be enrolled in the Jacksonville University Student Medical Insurance Plan and the premium for coverage is added to the tuition billing upon registration. This plan is also automatically added for Orthodontics students, excluding those participating in the Clinical Research in Orthodontic Fellowship Program. Coverage can be waived if evidence of other comparable coverage is presented prior to the waiver deadline.

Important Information for 2017-2018

National Guardian Life has been selected as Jacksonville University’s 2017-2018 Student Health Insurance provider. The premium rates are provided below. The 2017-2018 Plan brochure will be published as soon as it becomes available.

General Rules for Enrollment

Students may enroll after the deadline only if there has been a significant life change (i.e. loss of prior coverage) and should contact the Jacksonville University Student Health Insurance Administrator at (904) 256-7080 or health@ju.edu. A written request must be submitted to the Controller’s Office with documentation as to why they no longer are insured, along with proof of dropped coverage from the prior carrier. The rate will be pro-rated based on the month the student enrolls.

Effective Dates

The effective dates of benefit coverage for the 2017-2018 academic year are August 1, 2017 through July 31, 2018.

Special Note for International Students

Your insurance company must be based in the United States. International policies are not accepted. Your Plan must cover you from August 1st 2017 to July 31, 2018.

Policy for Waiving the Insurance

In order to waive the Jacksonville University Student Health Insurance Plan, your private health insurance must provide comparable coverage. This means your plan needs to have doctors and hospitals in Jacksonville, Florida; cover you for services beyond emergency care which includes pharmacy, physician visits, mental health, surgery, diagnostic testing, maternity, preventive services; and does not have a high deductible. The insurance company must be based in the United States. International policies are not accepted. Your Plan must cover you from August 1st 2017 to July 31, 2018.

Waiver Procedure

All eligible students who have other comparable insurance coverage must file a waiver by the deadline below in order to waive participation in the Student Medical Insurance Plan.

To waive the student health insurance you will need to complete the following steps:

  1. Go to www.gallagherstudent.com/Jacksonville.
  2. Click on "Student Waive."
  3. Create an account to log in.
  4. Complete the waiver form. Have your current health insurance ID as card ready you will need this information in order to complete the waiver form. You will be asked to review the information provided and click 'submit' to complete the process. Immediately upon submitting your Online Form you will receive a reference number. Please note and keep this information for your record.

The deadline to waive the school plan is Friday, February 02, 2018.

If the waiver information has not been submitted online by the indicated deadline, the student will remain enrolled in the University's Student Medical Insurance Plan, and the charge for the Student Medical Insurance Plan cannot be removed.
